Subject: SAFETY NOTICE-Information Sharing-Observation from Terminal inspection at Shell Bukom SBM, Singapore.

During recent operation at Shell Bukom SBM, one of our VLCC received following observations from terminal:

Observation no 1 - It was observed that vessel accommodation W/T doors were kept open during discharging operation:
- Upper deck (Port & Stbd) - B Deck (Port Side) - A Deck (Port Side) -Paint Store, Upper deck (Aft), entrance to engine room. W/T was shut immediately after duty officer was informed.
Observation no 2 - It was observed that vessel's crew member was using a mobile phone at B-deck port side, outside of vessel accommodation stairway platform.

Both the observations are non compliance of basic safe practices of tanker operation. To ensure these practices are prevented on your vessel please be guided by the following.
FOR OBSERVATION NO 1

In the given case some of the crew member went out from accommodation and left the door in open condition. This indicates lack of awareness by some of the crew members. Latest edition of ISGOTT chapter 24.1 (Page 345) contains the details on keeping external doors closed at certain times.
Company form OT/S/D/1009/122 - SHIP/SHORE SAFETY CHECKLIST item no 17 requires these doors to be closed prior commencement of cargo operation and to be rechecked every hour to ensure compliance with above. Any doors left open by any crew members will be closed during hourly check which is not acceptable. Therefore, it is important for all shipboard staff to be familiar with such requirement rather than only the staff involved with cargo handling.
To implement the requirement following to be done onboard:
a) Immediately conduct a crew meeting to discuss the issue using ISGOTT and Ship Shore Safety Check List.
b) Post following on internal and external sides of each door - "THIS DOOR IS TO BE KEPT CLOSED DURING LOADING, DISCHARGING, BUNKERING, TANK CLEANING AND GAS FREEING OPERATIONS"
c) Minimize the use of doors to only one while in port.
d) Discuss this issue in each pre-arrival meeting to the crew members indicating which door is to be used for the port operation.
e) Encourage crew members to close any open door and report to the CCR immediately.
FOR OBSERVATION NO 2

One of the crew member went outside of the accommodation in search of stronger mobile signal while using the mobile phone indicating lack of awareness. Latest edition of ISGOTT Chapter 4.8.6 (Page 77), Chapter 11.3.5.2.j (Page 193) & Chapter 24.10.1 (Page 353) contain details on requirement and guidelines in this regard.
Company form OT/S/D/1009/122 - SHIP/SHORE SAFETY CHECKLIST item no 38 is designed to implement above requirements which is coded with 'A' & 'R' in the form. Warning signs are posted near the gangway to warn visitors on correct procedure of use of mobile phones on board the vessel. However, if ship's crew members are not aware of the procedure then it will not be implemented on board. This may also be a reason of leaving external doors open as the crew member engrossed in mobile conversation is most likely to forget to close the door.
To ensure awareness among the crew members, following are to be done on board:
a) Immediately conduct a crew meeting to discuss the issue using the highlighted sections of ISGOTT and Ship Shore Safety Check List.
b) Post following on internal side of each door - "NO MOBILE PHONE ALLOWED BEYOND THIS POINT"
c) Discuss this issue in each pre-arrival meeting to the crew members highlighting the procedure for use of mobile phone on board the vessel.
All Safety Notice should be discussed in the Safety Committee Meeting and recorded in the minutes of meeting. This Safety Notice shall be posted at the common Notice Board accessible to all ship staffs.
