အဏၰ၀ါရဲ့ သေဘၤာတစ္စီးစစ္ေဆးေရး(PSC)က်ရွံဳး

သေဘၤာနာမည္အတိက်ေျပာရယင္ ယူနီကြန္းပါ
ဘယ္သူေတြကိုျဖဳတ္မလဲဆိုတာေစာင့္ၾကည့္ရမွာပဲ့
ဒီအခ်က္ေတြထိသြားတာ

Please note that during recent PSC inspection onboard one of our vessel at Nakhoka, Russia, flowing observation noted.
Deficiencies as under:
1.            Supplement to IOPP, Form B - page 3,2A.Oil Fuel tank protection (regulation 12A) not checked as required (2.A.1 or 2.a.2).
2.            Supplement to IOPP, form B-page 4 found in photocopy.
3.            No procedures implements with some type of waste (fluorescent lamps, Battery, Medicine etc.), But store in CCR, In Duty Mess Room.
4.            Fire protection self - closing door (Accommodation interior stairway B,C door not closing fully.
5.            Some of navigation charts for engaged voyage found in scan copy
6.            Scan copy charts are not up - date by 1237(T)/2006.
7.            Magnetic compass deviation not properly adjusted.
8.            Magnetic compass found with air big bubble.
9.            Magnetic compass illumination in a bad condition.
10.          FWD anchor light (spare) out of order.
11.          steering gear l.o gravity tank air vent cover securing bolt missing.
12.          D/Generator exhaust air vents protective gratings (funnel deck)Found dirty (oil on gratings and on the deck)
13.          In emergency room found unapproved battery charger (Electrical cables, socket was hanging on pipes)
14.          Emergency generator room fixed battery charger electrical cable not secured.
15.          Foam applicator unit plastic can (one) damage (Foam solution quantity less than 20 Lt)
16.          Food waste drum cover not ensured tightened covered as required by GMP, Item 10.5 (c).
17.          Steering gear hatch cover not fixed in open position due to hole less than diameter of securing bolts.
18.          tanks washing system water pipe flange bonding cable damaged.
19.          Fixed deck foam fire extinguisher system balanced pressure proportional with much stains of leakage.
20.          Balanced Pressure proportioner  not adjusted foam concentration ratio.
21.          Much of oil strains found on incinerator sludge tank body, on top, on ceiling above the sludge tanks, due to accident, but not records found in E/R log book, or Deck log book.
22.          incinerator sludge tank detectors (Overfill and sludge pump auto - stop) out of order (Level of sludge higher than position of detector).
23.          Main engine exhaust pipe inlet valve open/close indicator electrical cable disconnected (to composite boiler).
24.          Fuel oil pipe sighting glass from purifier to setting tank dirty, oil leakage through flange connection.
25.          E/R portable foam fire extinguishers (two)seal missing)
26.          Electrical cable (above M/E scav box) excessively dirty by stains of salt water.
27.          E/R Low platform plating not secured (part of emergency exit way with ledge).
28.          throughout of E/R found pressure gauge with stains of leakage (M/E fresh water cooler, no.1 & no.2 I.G scrubber cooling S.W pump, no.1 main cooling S.W pump etc.)
29.          E/R lower platform alarm panel partly damaged.
30.          Maintenance and inspection of fire protection system and equipment inadequate.
31.          Above deficiencies mainly caused by lack of maintenance , maintenance routine below standard.
32.          ISM system does not ensure adequate maintenance of ship and equipment.


Views above 32 deficiencies , office require all our fleet vessels to go through each observation, verify & rectify if any of these deficiency exist onboard your good vessels so that similar incident does not occur onboard any of our fleet vessels.
